Vision Symptom Questionnaire

What is the CISS? The Convergence Insufficiency Symptom Survey is a validated clinical questionnaire used to measure the frequency and severity of visual symptoms experienced during near tasks such as reading, writing, or screen use. While originally developed to evaluate convergence insufficiency - a condition in which the eyes have difficulty working together at close distances - it is widely used by functional and developmental optometrists to screen for a broad range of binocular vision disorders, including accommodative dysfunction, oculomotor deficits, and vergence anomalies. A higher score indicates a greater symptom burden that may benefit from clinical evaluation.

What is the BIVSS? The Brain Injury Vision Symptom Survey is a validated 28-item questionnaire developed to identify and quantify vision-related symptoms in individuals with a history of traumatic brain injury (TBI), concussion, or stroke. It covers eight visual domains: clarity, comfort, double vision, light sensitivity, dry eye, depth perception, peripheral vision, and reading. Because you indicated a history of brain injury, this additional survey has been included to better understand how your vision may have been affected.
